• Trade violates funded rules (risk limits, sizing, open positions, protection triggers)

• Token has low liquidity / high slippage conditions at that moment

• Network/RPC congestion or transaction expiry

• Terminal UI changes causing Tap to misread trade inputs (rare, but possible with overlay systems)
